I have volunteered to participate in a program presented by Newburyport Wellness LLC, Seaside Family Acupuncture and Healthy Mamas for Happy Families, which shall be referred to in this document as the Radiant Pregnancy Collective. I understand that I am joining a prenatal and postpartum educational series that is not intended to replace individualized sessions with practitioners or standard medical care and that all information provided will be generalized information, not individualized recommendations or health plans. I do here and forever release and discharge and hereby hold harmless The Thriving Through Pregnancy and Beyond Collective and their respective agents, heirs, assigns, contractors, and employees from any and all claims, demands, damages, rights of action or causes of action, present or future, arising out of or connected with my participation in this program including any injuries.

I acknowledge that the possibility of certain unusual physical changes during movement, meditation, exercise and modifying diet and supplements does exist. As with many health related services, there are certain potential complications which may arise during the receipt of these services. Those complications range from discomfort through serious health concerns requiring emergency medical care. These changes include abnormal blood pressure; fainting; disorders in heartbeat; heart attack; and, in rare instances, death. I understand that as a result of my participation in a group nutrition/movement program, I could suffer an injury or physical disorder that could result in my becoming partially or totally disabled and incapable of performing any gainful employment or having a normal social life. I confirm that I have no medical conditions that preclude me from participation in this program. I acknowledge and agree that it is my responsibility to inform my medical doctors of all exercise/supplements/diets that I will be partaking in so that he/she can make sure there are no contraindications to your medicine and health plan.

I acknowledge and agree that I assume the risks associated with any and all activities and/or exercises in which I participate. I acknowledge and agree that no warranties or representations have been made to me regarding the results I will achieve from this program. I understand that results are individual and may vary.
